Providers: Complete the Service Provider Data Collection form. Step #2 - Pay the listing fee The NTMHP Directory depends on donations to offset the operating expenses of the organization, such as for this website. A $ 30 donation for providers and $ 40 for facilities is requested to cover expenses of NTMHP and can be made online using PayPal, or by sending a check to the address below. This is an annual membership, which begins in April of each year.
If an invoice is required for payment processing, please let us know so we can send you one.
Once the PayPal process is complete, you will receive a Confirmation number for the
transaction with PayPal, and an email with your payment details. You will also have the
ability to print your payment receipt. It's easy!
